Ham, Vegetable & Lentil Stew

  1. Combine all ingredients except cheese in 4-quart slow cooker.
  2. Cover; cook on High heat setting 3-4 hours.
  3. To serve, place 2 tablespoons cheese in each individual soup bowl.
  4. Ladle soup over cheese.
